Testis. The testicular lobules contain spermatogonia, spermatocytes, seminal fluid, and hormones. Their secretions are carried by the seminiferous tubules to the efferent ductules of the epididymis. The testicular artery runs from its origin inferior to the renal artery parallel to the spermatic cord into the mediasti-

num of the testis, radiating with its branches into the testis. Such a transmediastinal artery can be seen as a linear anechoic band within the testis. Mediastinal septae reflect echogenic bands.

Epididymis. The epididymis lies on the lateral aspect of the testis and consists of a superior head and an inferior tail, which is continuous with the vas deferens. The latter duct ascends from the scrotum in the spermatic cord, accompanied by the testicular artery and vein and the deferential artery and vein. The vas deferens crosses over the ureter and passes behind the seminal vesicles to enter the upper part of the prostate (Figs. 12.25, 12.26, 12.27).

Testis, Epididymis

Fig. 12.26 Ultrasound scans through a normal testis and epididymis.

a Scan through the upper part of the testis (HO) displays the head of the epididymis (NH) and the patchy hyperechoic spermatic cord with the vas deferens.

b Midlevel scan through the right testis demonstrates a finely granular echo texture (same appearance as in uncomplicated orchitis). RE HO = right testis.

c Low longitudinal scan displays the lower portions of the testis and the tail of the epididymis within the fluid medium of the scrotum.

Fig. 12.27 Ectatic vas deferens (arrow) proximal to its entry into the prostate. HB = bladder; PR = prostate.

a Lower abdominal transverse scan: small, anechoic, avascular mass.

b Duct system (arrows) terminates at the prostate and has no connections with the bladder.
