против 3,7%), СРР (4,5% против 1,0%) и изолированным
амплитудным критериям QRS для ГЛЖ (26,3% против 4,6%)
соответственно. Все различия были статистически значимы
(р ≤ 0,001).
G. Finocchiaro с соавт. (2017), обследовав более 1000
высококвалифицированных спортс менов, показали, что
амплитудные критерии ГЛЖ присутствовали у 14% женщин
по сравнению с 42% мужчин, отклонение оси сердца влево –
у 0,4% и 4% соответственно. Наоборот, инверсия зубца Т
была более распространенной у спортс менок (9% у женщин
против 4% у мужчин). Эти результаты были подтверждены
исследованием А. Malhotra с соавт. (2017), где инверсия Т
отмечалась у 6,5% спортс менок против 2,1% мужчин.
Было также показано, что нарушения реполяризации,
свойственные чернокожим спортс менам-мужчинам, редко
наблюдается у высококвалифицированных спортс менок
(Finocchiaro G., 2018).
Можно заключить, что у мужчин-спортс менов признаки
электрофизиологического ремоделирования сердца развиваются чаще, чем у женщин. Собственно, и морфологическое
ремоделирование сердца у них более выражено.
В рамках темы монографии следует подчеркнуть, что
если при интерпретации ЭКГ у спортс мена клинические данные, в том числе спортивного анамнеза, неизвестны, то следует пользоваться не клинико-морфологическими, а электрофизиологическими терминами: высокий вольтаж QRS,
удлинение интервалов, нарушения процессов реполяризации
и т. п.
Таким образом, обзор литературы последних лет показал, что несмотря на то, что факт изменения электрокардиограммы под действием физических нагрузок является доказанным, сегодня не все вопросы о том, какие изменения
являются вариантами нормальной ЭКГ у спор
чательно решены.
В заключение главы следует сказать, что на сегодняшний
день факт существования электрофизиологического ремоделирования миокарда под действием физических нагрузок
тс менов, окон-

является доказанным. Электрофизиологическое ремоделирование происходит параллельно с морфологическим ремоделированием миокарда и во многом с ним связано. К нормальным изменениям ЭКГ под действием физических нагрузок,
по данным современной литературы, следует относить:
ЭЛЕКТРОФИЗИОЛОГИЧЕСКОЕ РЕМОДЕЛИРОВАНИЕ
СЕРДЦА ПРИ ФИЗИЧЕСКИХ НАГРУЗКАХ В НОРМЕ:
изменение электрической оси, ширины и морфо-
логии зубца Р;
изолированные амплитудные критерии гипертро-
фии левого желудочка сердца;
отклонение электрической оси вправо;
неполная блокада правой ножки пучка Гиса;
постоянная фрагментация QRS-комплекса без его
уширения;
синдром ранней реполяризации;
инверсия зубца T у юных спортсменов младше
16 лет («ювенильные Т»);
типичные нарушения реполяризации у черноко-
жих спортсменов;
синусовая брадикардия;
синусовая аритмия;
синусовые паузы 2000–3000 мс – у взрослых
и 1500–1880 мс – у детей;
предсердные эктопические ритмы, в том числе
нижнепредсердный ритм и миграция водителя
ритма по предсердиям;
а-в блокада первой степени и а-в блокада II сте-
пени (тип Мобитц I – периодика Венкебаха).
Данные критерии электрофизиологического ремоделирования следует трактовать как проявление нормальной адаптации к физическим нагрузкам при определенных
условиях (в частности, исчезновение при учащении ритма
сердца), которые изложены в данной главе, поскольку любой

из названных критериев может быть также и проявлением
патологии сердца, в том числе и жизнеугрожающей. При
наличии у спортс мена симптомов сердечных заболеваний
или предрасположенности к ним, а также ранних внезапных
сердечных смертей в семье, нормальная ЭКГ не должна препятствовать дальнейшей оценке сердечно-сосудистого риска
и обследования.
Несмотря на противоречия в литературе по ряду вопросов нормальной ЭКГ при адаптации к физическим нагрузкам, существуют и постоянно обновляются международные
стандарты интерпретации ЭКГ у спортс менов, а также отечественные нормативные документы, которые будут рассмотрены в следующих главах.
