Контроль над эффективностью терапии АБЛА осуществляет
ся преимущественно рентгенологическими и спирометрически
ми методами. Оценка серологических параметров также имеет
важное значение, поскольку определяет необходимость проведе
ния повторных курсов антифунгальной терапии еще до начала
выраженных клинических проявлений. Следует помнить, что
обострение симптомов бронхиальной астмы после достижения
ремиссии АБЛА может быть обусловлено не только обострением
АБЛА, но и другими факторами, такими как острые вирусные
инфекции. В таких случаях проведение антифунгальной терапии
не показано. Другие показатели, такие как эозинофилия, играют
вспомогательную роль, однако рост числа эозинофилов наряду с
клинической симптоматикой может предполагать наличие обо
стрения АБЛА. Уровень общего IgE на фоне лечения кортико
стероидами снижается, но обычно остается высоким даже в пе
риод ремиссии.
Для купирования обострения в тяжелых случаях АБЛА, особенно, когда применение системных КС не дает должного результата, могут быть применены методы гравитационной хирургии
крови (плазмаферез) [Кулешов А. В. и др., 1997]. Мы проводили
плазмаферез одному больному с тяжелой формой АБЛА с множественными инфильтратами в легких, тяжелым астматическим
состоянием, высоким уровнем преципитирующих антител к
A. fumigatus, при отсутствии эффекта от применения больших доз
системных КС. Проведение курса плазмафереза привело к быстрому положительному клиническому эффекту: прекращению
приступов удушья, улучшению лабораторных показателей. Нор
мализовалось количество эозинофилов в крови, СОЭ, снизились
титры антител к Af. Больной был выписан в фазе ремиссии с ре
комендацией приема преднизолона в дозе 10 мг/сут через день.
Обострение процесса отмечали через 6 мес после самостоятель
ного прекращения больным приема КС. Повторное проведение
курса плазмафереза вновь стабилизировало состояние больного
на срок до 6 мес.
В настоящее время ведутся разработки по иммунотерапии
АБЛА. Омализумаб, моноклональные антитела к IgE использова
ли для лечения АБЛА у детей с муковисцидозом. Исследование
включало семь детей с плохо контролируемым АБЛА, которые
показали значительное улучшение в ходе этой терапии [van der
Ent С. et al., 2007]. Омализумаб не применяли у больных АБЛА

без муковисцидоза, ничего не известно относительно безопасно
сти связывания большого количества IgE.
Диспансерное наблюдение за больными АБЛА (от 2 до
10 лет) выявило, что основными лабораторными показателями,
определяющими активность процесса при АБЛА, являются
эозинофилия периферической крови и динамика титров IgG к
Af. Такие показатели, как уровень общего IgE и IgE к Af, в мень
шей степени отражают динамику активности этого заболева
ния. Хотя в ходе длительной терапии системными КС уровень
общего IgE несколько снижался, он никогда не достигал нор
мальных показателей. Важным аспектом диспансерного на
блюдения является исследование в динамике данных спиро
метрии; главным образом ЖЕЛ и ОФВ
, что позволяет оценить

общую эффективность терапии, так как развитие фиброза и
дыхательная недостаточность являются главными факторами
инвалидизации больных АБЛА.

Заключение
Грибы рода Aspergillus являются условными патогенами. Их па
тогенный потенциал реализуется при нарушениях в иммунной
системе хозяина, а также при наличии определенной генетиче
ской предрасположенности. Детальный генетический анализ и
анализ состояния иммунной системы больного позволят лучше
проводить дифференциальную диагностику клинических форм
аспергиллеза и адекватное лечение. Своевременная коррекция
иммунитета может являться одним из способов предупреждения
инвазивных форм аспергиллеза, развивающихся на фоне иммуносупрессии, как хронической, так и транзиторной. Лечение аллергических заболеваний, вызванных распространенными пищевыми и респираторными аллергенами, может предотвратить
формирование гиперчувствительности к грибам рода Aspergillus.
В целом хочется обратить внимание врачей на значительное количество случаев, когда диагноз «аспергиллез» устанавливается
со значительной задержкой. По-видимому, распространенность
аспергиллеза значительно шире, чем считается на настоящий
момент.
Развитие лабораторной диагностики аспергиллеза остается
одной из первостепенных задач. Однако накопленные на насто
ящий момент знания по механизмам патогенеза, диагностике и
лечению аспергиллеза позволяют надеяться на повышение час
тоты своевременного выявления и адекватного безопасного ле
чения различных форм аспергиллеза современными препарата
ми, что приведет к сохранению и значительному улучшению ка
чества жизни многих больных.
