Больная, 52 лет, поступила в гинекологическое отделение с жалобами на ноющие боли в гипогастрии, увеличение живота, быструю утомляемость, слабость. Постменопауза 4 года. 5 лет у гинеколога не наблюдалась. Живот увеличен в размерах, при пальпации безболезненный, определяется симптом флюктуации; при перкуссии в положении на спине определяется притупление в боковых каналах живота, границы которого изменяются при изменении положения тела. Физиологические отправления в норме. При гинекологическом исследовании: наружные половые органы, влагалище, шейка матки без особенностей. Тело матки в правильном положении, плотное, подвижное, безболезненное, нормальной величины. Слева придатки не определяются, область их безболезненна. Справа определяется плотная опухоль с четкими контурами, безболезненная, размерами 14х12х10 см. Своды глубокие, выделения светлые. Диагноз? План обследования и лечения больной?

Postmenopause for 4 years. She did not attend the gynecologist for 5 years. The abdomen is enlarged in size, painless on palpation, the symptom of fluctuations is noted; with percussion in the position on the back, dullness in the lateral canals of the abdomen is determined, the boundaries of which change when the position of the body changes. Physiological habit is normal. On gynecological examination: the external genital organs, the vagina and the uterine cervix without any peculiarities. The uterine body is in normal position, firm, fixed, painless, of normal size. The left appendages are not palpable, their area is painless. On the right, a firm painless tumor with clear contours is palpated, of 14x12x10 cm in size. The vaginal fornices are deep, transparent vaginal discharge. What is the diagnosis? How would you investigate and manage the patient?

При гинекологическом исследовании: наружные половые органы, влагалище, шейка матки без патологических изменений, матка и придатки отдельно не контурируются, весь малый таз занимает плотная бугристая неподвижная опухоль, болезненная при пальпации, влагалищные своды укорочены, в заднем своде определяются «шиповидные выросты», выделения обычного вида. Диагноз? План ведения?

On gynecological examination: the external genital organs, the vagina and the uterine cervix are without pathological changes, the uterine body and the appendages are not separately contoured: the entire small pelvis is filled by a firm, irregular, fixed tumor that is painful on palpation: the vaginal fornices are shortened, in the posterior one "spike-like outgrowths" are determined; usual vaginal discharge. What is the diagnosis? How would you manage the patient?
