Coalition Building within Communities

The services and programs this approach engenders tend to develop around the circumstances and needs of local people, rather than the reimbursement patterns of the current system. Under the umbrella of improving the health of elders, these programs often establish new linkages – among providers; within health professions education institutions; and among health, human service, and community development agencies – to have impact on the lives of elderly in their communities. They build on the support of traditional institutional partners (hospitals, colleges and universities, clinics, social service agencies) and community stakeholders (businesses, churches, schools, local volunteers, and service organizations) to address the needs and expand the capacity of local elders.

Better Use of Existing Resources

Rather than create new systems for services or additional organizations to address the health of elders, these models seek to improve the connections between existing services and the elderly - to do a better job with what is already available. Some programs focus on coordinating care for elders, smoothing the transition from skilled nursing care to assisted living in the home. Often these programs deliver "wraparound" services – assistance that is not strictly "medical," but necessary to maintain or improve health. Better use of multiple health providers is another key part of some programs. By coordinating among health professionals, these models ensure that a community's elderly have the level of care they need, when they need it.

Expanded Community Capacity

The net effect, however, is that these programs often improve more than the lives of older citizens. For example, most elders prefer to "age in place" for as long as possible. By designing programming around the needs and preferences of this group, communities are able to keep elders among them longer – and often retain an important part of the local economy. Those who began with the intent of helping the elderly have found that community life in general benefits – whether improvements take the form of infrastructure, jobs and economic development, local leadership development, or the formation of viable coalitions for problem solving. When communities focus their resources to address one segment of the population in a comprehensive way, they often accomplish much, much more.
