Night watching
During the training the student has to be on the night watching in the therapeutic department two times. His duties include:
Daily round of the wards together with the doctor on duty.
Examination of the severe case patients being under the doctor’s on duty supervision.
Rendering urgent medical aid to these patients and also to the patients who had a sudden deterioration during their illness.
Reception of the patients according to the emergency indications, description of the patients’ states at admission and the rendered medical aid.
Consulting if it is necessary with the doctors of other specialties (surgeons, neuropathologists, etc.)
Participation in diagnostic procedures and medical measures.
Filling in case-histories (in severe cases – every hour) with data concerning all the changes of the patient’s condition, including suddenly arising ones, and data concerning the therapeutic measures.
Making a report on the watching before the head doctor and during the morning five-minute medical conferences.
Acquisition of the working skills in the extreme conditions.
Examination of the patients, making the preliminary diagnosis, prescribing of survey and treatment to the patients brought to a therapeutic in-patient department according to emergency indications (by an ambulance car or an emergency team).
Carrying out scheduled and urgent therapeutic measures:
giving hypodermic, intramuscular injections, intravenous injections
drop-by-drop introduction of medicinal substances
cupping
-gastric lavage
enema application
preparation for radiological researches of internal organs.
Rendering urgent medical assistance at emergency conditions.
The student has to make a detailed report about the night watching with a brief description of the case-histories of the patients under the supervision and the newly hospitalized patients.
Work at the polyclinic
The overwhelming majority of patients undergo treatment at a polyclinic so a great value of this link of therapeutic service is obvious.
At a polyclinic the students perform the following scope of work:
Make the acquaintance with organization of work at the polyclinic registry.
Study work organization in a therapeutic consulting-room of the polyclinic, quantity of daily seen patients, get acquainted with the equipment, staff list, working schedule of medical personnel, qualitative and quantitative indicators of medical establishment according to report documentation.
Students together with a doctor receive patients, fill in necessary documentation. Special attention is paid to accuracy of records in an out-patient card. 6-8 patients are received daily.
Students during reception hours carry out examination of a patient, make diagnosis, discuss a course of the treatment, preventive measures, questions of temporary invalidity, hospitalization of patients, and direct them to consultations to the other specialists.
Students participate in organization and carrying out the primary, secondary and tertiary preventive measures.
They get skills of work at the polyclinic. The student should be able:
to examine an out-patient
to fill in an out-patient card
to make out directions (slips) to diagnostic and auxiliary consulting rooms and also to consultations
to estimate temporary invalidity
to write out prescriptions for narcotic substances and preparations equal to them.
