Заключение
В истории региональной анестезии получение парестезии было классическим методом локализовать нерв. Большинство трудностей применения РА, в частности у детей, преодолено за счет использова­ния методик, не требующих извлечения парестезий для локализации нервных стволов.
Фармакологические свойства современных местных анестетиков полностью исследованы даже для новорожденных. В нескольких крупных исследованиях периферической РА у детей [4, 62, 99] пока­зана необоснованность страхов относительно повреждения нервов.
Основными преимуществами РА является то, что уменьшается об­щая доза опиоидных анальгетиков, устраняется опиатная депрессия дыхания и тем самым нет необходимости в ИВЛ; пациент с полным желудком подвергается значительно меньшей опасности; улучшает­ся циркуляция в области сомнительной жизнеспособности тканей; тошнота, рвота и другие осложнения общей анестезии встречаются значительно реже; амбулаторные больные могут быть раньше выпи­саны домой.
Главным преимуществом РА является длительный период после­операционного обезболивания. Пробуждение ребенка происходит без боли. Длительность послеоперационного обезболивания может быть продлена, используя методику катетеризации фасциальных пространств.
Удовлетворенность родителей качеством послеоперационного обезболивания при использовании региональной анестезии у их де­тей очень высокая.
РА позволяет сделать анестезиологическое пособие соответству­ющим степени операционного вмешательства, а не избыточным, что иллюстрируют операции на кисти — относительно малотравматич­ные, но с большой длительностью.
Аксиллярный блок является простым в выполнении и характери­зуется относительной безопасностью. Использование специальных

игл для региональной анестезии уменьшает риск повреждения не­рвов [110].
Тем не менее имеются и слабые стороны РА.
1. Невысокая частота полной блокады [56, 105, 112] препятствует широкому распространению аксиллярного блока и связана с тем, что некоторые нервы часто не могут быть блокированы. Так возникает необходимость в проведении дополнительных блоков.
2. Длительный латентный период — необходимо ждать 20–30 ми­нут до развития полного блока. Решение данной проблемы организа­ционное: выполнять блок за 20–30 минут до поступления пациента в операционную комнату.
Основных методик выполнения аксиллярного блока четыре. Тем не менее для детей рассматриваются только две методики: перива­скулярный метод (методика «клик»-признака) и методика перифери­ческой нейростимуляции. Эти методики не требуют наличия созна­ния и могут выполняться в том числе в педиатрической практике.
Успех регионального блока зависит от правильного позициони­рования иглы к желаемому нерву. Дополнительно к чувству прокола фасции и пульсации иглы методика с нейростимулятором дает ане­стезиологу еще один надежный признак убедиться в правильности позиции кончика иглы: по мышечному сокращению на электриче­ский импульс. Тем не менее нет окончательных доказательств пре­восходства одной методики над другой.
