Fig. 6.27  Subxiphoid four-chamber view

2.23.1\ Scanning Method

The patient lies flat or on the left-anterior position. The transducer is placed in the subxiphoid (subcostal) area with some superior titling of the beam towards the head to take a coronary section of the heart.

2.23.2\ Section Structure

The structures imaged are the left atrium, right atrium, left ventricle, right ventricle, interatrial and interventricular septum, mitral valve, and tricuspid valve.

2.23.3\ Measuring Method and Normal Value

Transverse diameters of the heart chambers can be taken down in this view. The diameters of left and right ventricle measured in diastole at the

mitral chordae tendineae level are 40 ± 10 and 35 ± 10 mm. The diameters of left and right atrium measured in systole at the atrial middle level are 37 ± 5 and 37 ± 6 mm.

2.23.4\ The Clinical Application Value

When the images are disturbed by the air in lung during a routine precordial region scan, the subcostal view could be used instead to observe the heart. Because of the beam being perpendicular to the septum and with little echo dropout, the view is usually used to diagnose the atrial abnormalities, such as atrial septal defect, so this view is a supplementary section for diagnosing children’s congenital heart disease and observing the diaphragmatic surface pericardium and pericardial effusion.
