12  Gynecologic Ultrasound Scanning

465

3.5.1\ Method

The patient should have a full urinary bladder. Probe is placed in the abdominal wall of the hypogastrium, and scan is carried out from multi- angle and multi-plane. The section of the maximum ovary can be obtained with the direction of sound beam through the bladder.

3.5.2\ Section Structure

The anechoic area inferior to the abdominal wall is the distended bladder, and the hypoechoic nodus left behind the bladder is the ovary; the

ovarian follicle in the ovary has disappeared at this phase.

3.5.3\ Measuring Method and Normal

The ovary is atrophied and becomes small in postmenopause; generally, its size does not need to be measured.

3.5.4\ The Clinical Application Value

Generally, ultrasound exam could not detect the ovary because of the ovary atrophy. It is normal that the ovary could not be detected.

4.1.1\ Method

The patient should void the urinary bladder. Probe is placed in the vagina, and multi-plane scan is carried out from sagittal section to coronal section. Located at the uterus bilateralis.

4.1.2\ Section Structure

The ovaries appear oval. There are many anechoic ovarian follicles in the ovaries.

4.1.3\ Measuring Method and Normal

During the follicular phase, the dominant follicle grows bigger with time gradually; the largest

diameter of the mature follicle may even exceed 2.0 cm in diameter.

4.1.4\ The Clinical Application Value

The continuous transvaginal ultrasound exam could monitor the growth and development of the dominant follicle and help to judge the ovulation time, so as to guide the treatment of infertilitas feminis.

4.2.1\ Method

The patient should void the urinary bladder. Probe is placed in the vagina; multi-plane scan is carried out from sagittal section and coronal section. The ovaries are located at the uterus bilateralis.

4.2.2\ Section Structure

The ovaries are shown as oval in this plane. The corpus luteum can be seen as round, solid, and heterogeneous in the ovary.

4.2.3\ Measuring Method and Normal

The size, structure, and echo of corpus luteum vary largely; the size measurement is not useful for clinical diagnosis.

4.2.4\ The Clinical Application Value

When the corpus lutea are detected by the ultrasound exam, it means that the ovaries had ovulated.
