9  Abdominal Vascular Scanning

3.2\ Common Hepatic Artery

Blood Flow Spectrum

Fig. 9.10  Common hepatic artery blood flow spectrum

3.2.1\ Scanning Method

The patient should be on an empty stomach for 8–12 h, and should be in a supine position. The probe is placed at the subxyphoid level, showing a longitudinal section of the common hepatic artery. The Doppler sample volume is put in the center of vessele cavity of the common hepatic artery. The measurement angle is less than 60°.

3.2.2\ Measurement Method and Normal Value

The pulse Doppler blood flow spectrum is shown, measuring the blood flow velocity and RI. The normal flow is 91.1 ± 24.9 cm/s. The hepatic artery blood flow appears with three upward peaks and a low RI.

3.2.3\ Clinical Application Value

(1)On this section, we can judge whether the hepatic artery is unobstructed and whether an anastomotic stenosis or thrombosis is present in patients who have received a liver transplant. If the color blood flow is interrupted or cannot be shown and the hepatic artery blood flow cannot be measured, this will indicate a vascular stenosis or occlusion.

(2)The hepatic artery blood flow velocity will be quick compensatory in the case of a primary hepatocellular carcinoma or portal venous obstructive disease, such as a portal vein thrombosis or tumor emboli or thrombosis, on this section. The color blood flow is interrupted or cannot be shown.
