If only может быть более эмфатической, чем I wish. Глагольные формы после if only такие же как и с глаголом wish.

If only относится ко всему предложению, выражающему нереальное желание, и подчеркивает малую вероятность того, что это желание будет реализовано. В таких предложениях часто используется модальный глагол would, который еще более подчеркивает малую вероятность выполнения желания. Глагол-сказуемое в таких предложениях употребляется в форме Subjunctive.
