is new edition of Essentials of Pediatric Urology has been extensively updated, revised and rewrit­ten to reect the many advances and innovations in Pediatric Urology since the publication of the second edition. is new edition has also been updated to reect the valuable information on long-term outcomes now emerging from follow­up studies.
e publication of a new edition and the appointment of an Anglo American Pediatric Urologist co-editor created an exciting opportu­nity to extend the international reach of Essentials of Pediatric Urology. is new edition therefore includes authoritative chapters contributed by leading North American Urologists in addition to chapters by new and existing contributors from the United Kingdom, France and Turkey.
e new, more multinational authorship of chapters created a dilemma regarding the pre­ferred use of spelling and terminology. Pediatric or Paediatric? Ureteric of Ureteral? Ureteropelvic or Pelviureteric? Aer careful consideration we decided against a standardized policy, opting instead to allow contributors to use the spelling and terminology with which they were familiar. We are grateful to our publisher for endorsing this decision. Essentials of Pediatric Urology is not intended to serve as a comprehensive source of reference for specialist Pediatric Urologists. Nevertheless, it provides authoritative coverage
of the complex anomalies managed by special­ist Pediatric Urologists as well as the urologi­cal conditions comprising a substantial part of the everyday workload of Urologists, General Pediatric Surgeons and Pediatricians.
We have strived to retain the accessible, gen­erously illustrated format which proved popular with readers of the rst two editions. roughout the book our over-riding aim has been to provide a concise and up-to-date account of the diagnosis and management of the urological conditions of childhood which is factual and evidence based.
e content of this new edition has been designed to meet the requirements of the Intercollegiate Fellowship examinations in Urology and Paediatric Surgery in the United Kingdom, the Fellowship of the European Academy of Paediatric Urology and the North American Board Examinations. With this in mind, the book includes a self-assessment section of multiple choice questions which is intended to serve as a valuable learning and revision aid.

INTRODUCTION
is chapter will concentrate predominantly on the clinical aspects of the embryological development of the genitourinary tract but will also refer to relevant advances in scientic methodology.
Techniques such as polymerase chain reac­tion (PCR), uorescence in situ hybridisation (FISH) and studies using transgenic “knockout” mice have greatly advanced our understand­ing of the genes involved in regulating normal embryological development. In addition, inno­vations in microscopy and three-dimensional (3D) reconstruction are shedding new light on structural development of the human embryo and fetus.

GAMETOGENESIS, FERTILISATION AND THE GENETIC BASIS OF UROLOGICAL ABNORMALITIES
In males, the formation of gametes (spermato­zoa) does not commence until puberty whereas in females it commences during fetal life but does not progress beyond prophase of the rst meiotic division. At this stage the primary oocytes enter a dormant phase until the onset of puberty when a small number of primary oocytes resume the long – arrested meiotic division. Under the inu­ence of follicle-stimulating hormone and luteinis­ing hormone, one or two secondary oocytes are extruded from the ovaries at the time of ovula­tion. Penetration of the protective zona pellucida of the oocyte by a spermatozoon triggers a second meiotic division to create a “denitive oocyte”.
