Text a.

One morning a victim of a street accident was brought to the traumotological department. He received a blow on the jaw and complained of a severe pain. The lower part of his face was swollen, there were many injuries, abrasions and bruises on his face, forehead and skull. The patient could not speak of pain.

The nurse on duty recorded his initial data which the patient wrote on the paper.

On examinations the doctor found a compound fracture of the lower jaw at the symphisis, with great mobility at the seat' of injury. It was necessary to fix the fracture immediately and the doctor directed the patient to the operating room. Two assistants holed him during this procedure. One of them fixed the patient's head in a steady state, the other averted the patient's lower lip. The doctor passed the drill through the bone on either side of the fracture. Care was taken not to injure the fangs of the teeth. The silver wire was then passed through the opening. Next, the tubular needle was withdrawn and with it the wire. This needle was to act as a director to the posterior opening of the aperture, and to obviate difficultly and delay in searching for the entrance. Afterwards the end of the wire was inserted into the slit of the key and twisted into three or four coils, the same being done with the end of the wire, until the fracture was fixed.

On the fifth day the doctor found that the wire had slackened the use of the key in one of the coils. Although the either end of the wire may be tightened under these circumstances, the doctor thought it better to choose the shorter coil, and, before proceeding to twist, he made sure that the cross piece of the wire was well set into the slit of the key. In this case the wire required tightening every three or four days. In twenty days the fracture was firm and united. The patient, from the very beginning, expressed his ability to masticate, which he was not permitted.

The application of the wire ligature need not occupy more than three minutes. The removal of the wire requires the next procedure: the doctor introduces the key into one of the coils and unwires it, afterwards passing the key into the remaining coil, and winding up, until all the wire has been coiled upon the stem of the key.

The condition of the patient was satisfactory during the whole course of treatment. The feeding was produced through a tube.

In a month he was discharged from the hospital.
