1. A 75-year-old male presents with a six-hour history of colicky lower abdominal
discomfort and distension. He tells you that he has not passed wind or opened
his bowel for the past 24hours. On examination he has an empty rectum. You
arrange for an erect abdominal X-ray which reveals dilation of the small bowel
and colon to the junction of the distal transverse colon and collapse distal
to this.
2. A 27-year-old man presents with a six-month history of intermittent abdominal
pain associated with diarrhoea. He reports a loss of appetite and a loss of four
kg during this time. He has no signicant medical history and takes no medica-
tion. On examination he has generalised abdominal tenderness but no guarding
or rebound tenderness, he also has aphthous ulcers in his mouth. Abdominal
X-ray demonstrates a featureless colon. Colonoscopy is arranged and a biopsy
is taken which shows a non-caseating granuloma on histology.
3. A 33-year-old male presents with colicky central abdominal pain for the
past 12 hours, shortly after it started, he had multiple episodes of bilious
