GuestEditors’Preface
Adult spinal deformity (ASD) is a clinical problem of increasing prevalence, and thus
physiciansandpatientsworldwideareawareofit.Withincreasinglongevity, thenormal
degenerationofthespinemayleadtovariousASDproblemssuchaslumbardegenerative
scoliosis with or without accompanying spinal kyphosis. In addition, ASD includes a
spectrumofpreexistent childhooddeformities,suchasscoliosisorkyphosis, thatslowly
progress to symptomatic stages over adulthood. Clinical manifestations may include
progressivedeformity,potentialspinalimbalance,andspinalstenosis,withresultantaxial
or lower extremity symptomatology. Health-related quality-of-life assessments often
demonstrate severe adverse effects of ASD that can interfere with many aspects of
physical, emotional, and psychological well-being. When clinical and radiographic
scenarioswarrant,surgicalintervention,rangingfromsimpledecompressionstocomplex
totalspinereconstructions,shouldbeconsideredinappropriatepatients.
Wehaveassembledaglobalpanelofspecialiststosharewithustheirexperienceinthe
management of ASD, from evaluation to treatment, and including such issues as
instrumentation and surgical techniques, as well as preventing and managing
complications.Thoroughpatientevaluation,bothmedicalandsurgical,iswarranted,with
patientselection forindicated surgicalinterventionone ofthe mainkeysto asuccessful
outcome.Pertinentissues,suchasbonedensityevaluationandpreoperativeoptimization,
must be addressed with the use of intraoperative adjuvants to ensure stable internal
fixation to the spinal column in patients requiring stabilization with or without
realignment. For patients with progressive deformity producing segmental, regional, or
globalmalalignment,variouscorrectivestrategiesarediscussedtosafelyrealignthespinal
columnusingvariousformsofspinalosteotomieswithadjuvantspinalinstrumentationto
secure the spinal segments in their realigned position. Spinal fixation techniques are
especially challenging when instrumenting the sacropelvic unit in long constructs. The
various forms of osteotomies utilized range from simple facet excisions to extremely
complex three-column osteotomies such as pedicle subtraction and vertebral column
resectiontechniquesthatareoccasionallyrequiredforpatientswithseveredeformitywith
accompanyingimbalance.Ensuringneurologicsafetyduring ASDsurgeryis paramount,
because these operations have an early neurologic complication rate that is not
insignificant andcanlead to permanent deficits. Allof these essential preoperative and
intraoperativefactorsarediscussedindetail.
Even with initial surgical success, the long-term success of surgery for ASD is
controversial. Various factors, such as wound infections, pseudarthrosis, and adjacent
segmentpathology,themostcommonbeingproximaljunctionalkyphosis(PJK),canlead
to deterioration of the clinical outcomes over time. The durability of clinical outcome

ASD patients, bothwith nonoperative and operative procedures, must beascertained to

justifythe widespectrumof interventionsavailable. Asinall areasof surgery, selecting

Wehopethisbook willhelpspinesurgeonsfromaround theworldnavigatetheoften
controversial and complicated clinical issues involved in the management of ASD
patients,sothattheoutcomecanbemaximizedandthecomplicationsminimized.
