1. The subject of the lesson: RESPIRATORY FAILURE
Study-hours: 4
The aim: The students must be able to find the causes of respiratory failure, determine the type, severity and be able to correct this abnormal situation by treating the underlying disease and using replacement therapy.
Educational goal: A. Understand the concept of respiratory failure. 2. To understand all possible mechanisms of respiratory failure. 3. Know the clinical manifestations of respiratory failure for its detection and assessment of severity. 4. Know the methods of inspection necessary to determine the cause, type and degree of severity of respiratory failure. 5. Know the approaches to the correction of respiratory failure.
The student must know:
1. To have the understanding of term «respiratory failure» and know distinction between pulmonary and respiratory failure.
2. To know the modern classification of respiratory failure (pathogenic, considering the speed of development and severity).
3. To understand the basic pathogenetic mechanisms of respiratory failure.
4. To have an idea about the researches necessary for the diagnosis of respiratory failure and clarify its characteristics.
5. To know methods of correction of respiratory failure.
The student must be able:
1. Identify the status of respiratory failure, its cause and the main characteristics on the basis complains, anamnestic data and results of objective investigation.
2. Work out a plan for additional methods of examination for confirmation of respiratory failure and elicitation its characteristics.
3. Write a treatment plan of patient depending on disease, which lead to respiratory failure, its type and severity.

To have an understanding of the mechanisms of gas exchange through the alveolar-capillary membrane, the biochemical structure of the surfactant and the importance of its structure for performing functions; the biochemical composition of bronchial mucus. |

Normal physiology |
To know the basic functions of respiratory tract; have an idea about the process of gas exchange, how respiratory system take part in the elimination of metabolic products, in thermoregulation, in maintaining acid-base balance; know neuro-hormonal mechanisms of regulation of respiration. Understand the importance of protective elements of respiratory system - mucocilliar clearance, surfactant, immune mechanisms (alveolar macrophages, neutrophils, lymphocytes, immunoglobulins, complement system, lysozyme, lactoferrin). |

Physiopathology |
Know basic pathogenetic mechanisms of respiratory failure (centrogenic, neuro-muscular, thoracic, abdominal, obstructive, restrictive) and clinical and laboratory characteristics these types of respiratory failure. Know the parameters for determining the severity of respiratory failure. |

Pharmacology |
To know the mechanism of action, indications and contraindications of medicines, which is used in treating of respiratory failure and be able to write them in the form of recipes. Know regimes, indications, contraindications, side effects and methods of performance of oxygen therapy and non-invasive or invasive ventilatory support. |
8. |
Propaedeutic therapy |
Demonstrate skills in examination of patients with respiratory failure (collecting complaints, history of disease and life, doing an objective examination, analyzing the results diagnostic testing (chest X-ray, CTscan, functional tests, CBC, general analysis of sputum, gases of blood).
