Is It Possible toCure Brain Tumors?
In recent years, research in the battle against cancer has yielded remarkable
advancements in various areas, thanks to the potential for earlier and more
precise diagnosis, accurate prognostic characterization, and the adoption of
new, increasingly effective, sustainable, and personalized therapeutic strate-
gies. However, these advancements have not been equally realized for all
types of tumors, including brain tumors, particularly glioblastoma, for which
improving outcomes, duration, and quality of life remains very challenging.
In fact, detecting brain tumors on time is highly challenging, except for
incidentalomas, as there are currently no widely recommended screening
methods for early detection. The initial symptoms of early-stage brain tumors
are often fortuitous and difcult to interpret. Similarly, identifying an initial
recurrence is not easy due to the presence of many non-specic features that
do not distinguish between disease relapse and post-therapeutic changes.
Neuroimaging has made signicant advancements with the introduction of
revolutionary computerized equipment such as Computed Tomography (CT),
Positron Emission Tomography (PET), and Magnetic Resonance Imaging
(MRI), which complement invasive radiological methods and electroenceph-
alographic techniques. Since the 1980s, further progress, facilitated by hybrid
machines, advanced functional and molecular imaging, technological evolu-
tion, articial intelligence, and new therapeutic approaches, has led to
improved classication and characterization of individual cases and the adop-
tion of better overall strategies. This progress is also attributed to the advance-
ment of preclinical and pharmacological research, which has contributed to
the development of targeted therapies. However, despite these advancements,
gliomas, particularly in patients with glioblastoma, remain challenging to
cure, and their prognosis remains grim.
This book is being published during an “interlocutory” phase in the ght
against brain tumors. Unlike other types of cancer, personalized therapies that
can effectively treat individual patients have not yet been validated. This is in
contrast to the progress seen in other types of cancer. As shown by a recent
systematic review on personalized therapeutic approaches to glioblastoma,
such personalized therapy has not been proven to signicantly ameliorate the
survival of patients with glioblastoma, and there is currently no therapy ready
for regular clinical use. Nevertheless, ongoing endeavors are focused on

advancing personalized medicine in the treatment of brain tumors, including
glioblastoma, through innovative strategies such as mRNA vaccines and
immunotherapies.
The book aims to inform readers about the latest tools for better character-
izing primary brain tumors and evaluating response. It also seeks to facilitate
the early detection of recurrence and assist surgeons and radiotherapists in
performing the most decisive interventions while prioritizing the patient’s
quality of life. These objectives align with the ongoing evolution of brain
tumor imaging, which plays a crucial role in improving preoperative diagno-
sis, predicting tumor grading and patient prognosis, planning surgery and
radiation therapy, and assessing treatment response.
The text also emphasizes the growing necessity for collaboration among
experts from various disciplines, enabling the combined use of advanced
diagnostic and therapeutic approaches. To assemble the most effective team,
it is essential to enlist leading professionals from all relevant elds, creating
a balanced blend of experienced authors and vibrant young scientists. These
individuals should possess the ability to merge creativity, expertise, culture,
and knowledge of the most cutting-edge tools for the adoption and explana-
tion of modern diagnostic and therapeutic techniques.
In our view, this approach represents the most promising way to embrace
the future, while maintaining a strong foundation in preclinical research and
an understanding of the history of science, which serves as the framework for
new discoveries.
Thus, to experts in the pathological and molecular classication of tumors,
we have added professionals skilled in the use of PET with new and more
established radiotracers, neuroimaging experts, surgeons, oncologists, and
radiotherapists. These experts present the latest advancements in their respec-
tive elds of expertise.
We are very proud, as coaches of this team, to have among the authors
some of the leading international authorities in the most important issues
involved.
Our hope is to be able to remove the question mark from our editorial and
rewrite a new edition of this book shortly in which the title of our introduc-
tory comment would be: “It is possible to cure brain tumors.”
P.S.: Egesta Lopci and Luigi Mansi want to thank, for their support, the
Palummo family, organizer since 2019 of the Carla Russo Prize, dedicated to
the memory of a glioblastoma victim, rewarding the best young researchers
contributing in the ght against gliomas. Our purpose, as editors, is to help
turn their pain into hope.
