Preface

Obstetrical emergencies can be among the most stressful events an emergency phy-
sician will face in their entire career. Late pregnancy bleeding or a precipitous emer-
gency department (ED) delivery can provoke anxiety in even the most seasoned
emergency physician. Trauma and cardiovascular emergencies in pregnant patients
can result in catastrophic outcomes for both mother and baby. When it comes to
pregnant patients even seemingly minor presentations, such as a trip and fall, can
become an area of treachery.
The goal of this book is to provide an evidence-based, practical approach to the
wide spectrum of obstetric complications an emergency clinician must be prepared
to manage throughout all trimesters of pregnancy, as well as postpartum. Since the
publication of the rst edition, the evolving political landscape has highlighted new
areas where emergency physicians must be prepared to act with condence and
skill. This second edition expands on the foundation of the rst by adding compre-
hensive new chapters on post-abortion complications, complications of assisted
reproductive technology, and postpartum emergencies such as mastitis and endome-
tritis. These topics reect both the expanding scope of emergency practice and the
reality that the rst clinician to evaluate and stabilize these patients may not be an
obstetric specialist but the emergency physician.
This book is written for emergency clinicians in all practice settings. Obstetrical
support services vary widely between facilities, and emergency physicians must be
prepared to initially manage and stabilize any pregnant patient who comes through
the door—whether in a tertiary care center with in-house obstetrics or in a rural ED
hours from specialty backup. Our contributing authors bring experience from a
range of practice environments, ensuring that the recommendations are practical,
adaptable, and grounded in the realities of emergency care.
It is my hope that this book serves not only as a resource for day-to-day patient
care but also as preparation for those rare, high-stakes situations that demand imme-
diate, decisive action. By combining up-to-date evidence with the lived expertise of
clinicians who have faced these emergencies rsthand, we aim to equip emergency
physicians with the knowledge, strategies, and condence to provide the best pos-
sible care for both mother and baby—no matter the setting or circumstance.
