About the Editors
PradeepChowbey is currently Chairman of the Max Institute of Minimal
Access, and Chief of Surgery and Allied Surgical Specialties at Max Hospital,
New Delhi, India. He is the Honorary Surgeon to the President of India and
Surgeon to His Holiness the Dalai Lama. He is the Founder President of the
Asia Pacic Hernia Society (2004) and Former President of the Asia-Pacic
Metabolic and Bariatric Surgery Society (2010–2012), of the Pacic Chapter
of the International Federation for the Surgery of Obesity and Metabolic
Disorders (2011–2013), and of the International Federation for the Surgery of
Obesity and Metabolic Disorders (2012–2013).
He established the Minimal Access, Metabolic & Bariatric Surgery Centre,
Sir Ganga Ram Hospital, New Delhi, in 1996, the rst of its kind on the Asian
subcontinent. His current institute, the Max Institute of Minimal Access, is
accredited as an International Centre of Excellence for Endohernia Surgery
and Bariatric Surgery. He was on the steering committee for formulating the
Worldwide Guidelines and Recommendations for inguinal hernia and ventral and incisional abdominal wall hernias published in the Surgical Endoscopy
journal (Springer).
He is the Editor in Chief of the Journal of Minimal Access Surgery, the
ofcial publication of IAGES, a Section Editor (Ventral/Incisional Hernia
Surgery) for Hernia: The World Journal of Hernia and Abdominal Wall
Surgery, and an Associate Editor for the International Journal of Abdominal
Wall and Hernia Surgery.
Davide Lomanto
University of Singapore and Director of the Minimally Invasive Surgical
Centre (MISC) and of the KTP Advanced Surgery Training Centre (ASTC),
National University Hospital (NUS). He is a Secretary-General and a Past
President of the Endoscopic and Laparoscopic Surgeons of ASIA (ELSA); a
Founding Member and Advisory President of the Asia Pacic Hernia Society
(APHS); and a Founding Member and Past-President of the Asia-Pacic
Metabolic and Bariatric Surgery Society (APMBSS). He has been awarded
Honorary Memberships in several endolaparoscopic societies: Japan (JSES),
India (IAGES), Indonesia (PBEI, PERHERI), the Philippines (PCS, PALES),
and Thailand (RCST). He recently been awarded the prestigious Honorary
Fellowship Ad Hominem from the Royal College of Surgeons of Edinburg
(FRCS) and from the European Association for Endoscopic Surgery (EAES).

The Economics ofHernia Surgery

1.1 Introduction
Hernia surgery is one of the most common operations being done worldwide. Statistics from the
different countries varies but in the US, China,
and India, inguinal hernia surgery can go as high
as one million cases per year [1]. Throughout the
lifetime of an individual, the chance of having a
hernia operation is 27% for males and 3% for
females [2]. Hernia surgery involves not only
inguinal hernia but also ventral or incisional hernias. Through the years, the management of hernias has evolved from tissue repairs to prosthetic
repairs and laparoscopic repairs. With the advent
of all of these technological breakthroughs and
new techniques in hernia surgery, it is necessary
to assess the economic impact of these things in
terms of health care cost, as well as costs that the
patients will incur.
1.2 Health Economics
In terms of health economics, different countries will have different systems of funding
health care, but they have one thing in common; health care resources are limited and
needs to be spent rationally for the benet of
more people. In line with this, health economists need to make crucial decisions as to
which will be funded and which will be
rejected. In the case of hernia surgery, there are
so many variables that need to be considered in
deciding the most economical choice for a hernia patient. Things that need to be considered
are the following:
1. Tissue repair vs. prosthetic repair
2. Open vs. laparoscopic repair
3. Type of mesh to use
4. Type of xation
5. Day case or in-hospital procedure
6. Type of anesthesia
7. Antibiotic use
8. Impact on patients’ quality of life and return
to work
With all these things to consider, it is essential
for the decision-makers of health care to understand the concepts of health economics. Decision
should be based on several concepts:
1. Cost-minimization analysis
2. Cost-effectiveness analysis
3. Cost-utility Analysis
4. Cost-benet analysis
