Is there a doctor on board?

You are midway through a routine flight, when suddenly a passenger collapses clutching his chest and struggling to breathe. This is one of the situations that cabin crew are trained to deal with and aircrafts are equipped for. So, what are some of the most common medical emergencies?

This event – a heart attack – is the leading cause of in – flight death, and the leading medical cause of diversions. Medical kits include aspirin and vasodilator spray to keep the blood flowing when there is a chest pain. In case of cardiac arrest, cabin crew are trained to give CPR, while many airlines now also carry defibrillators to restart the heart. Cardiac monitors are used increasingly, so that the data can be transmitted to medical advisors on the ground.

A large number of diversions are caused by injuries to passengers. Items falling from the overhead bins are a common cause of head injury, while unexpected turbulence can easily result in broken bones. First – aid kits are equipped with splints and bandages to stabilize limbs, as well as plasters for minor cuts.

Asthma is a common condition that can be life – threatening, especially when the sufferer’s inhaler is in the baggage compartment. In addition to oxygen, bronchodilators and adrenaline are kept in order to open up the airways.

Dangerous breathing problems can also result from severe allergic reaction, which worries airlines so much that some no longer serve peanuts. Most carry EpiPens, as well as antihistamine and adrenaline to prevent anaphylactic shock.

Most medical kits contain glucose and glucagon injections to treat passengers who suffer hypoglycaemic episodes. The disruption of regular eating habits can lead to dangerous a dangerous drop in blood sugar levels.

MEDICAL SERVICE

The crew members and the CAs when flying abroad must have a special document which is called “International Certificate of Vaccination”. This is to certify that a person has been vaccinated or revaccinated against the yellow fever, smallpox and cholera. The outgoing Russian citizens must also have such a document.

Foreign tourists arriving in Russia from any country where they have an epidemic of the typhoid fever, malaria, plague or some other infectious diseases must also have a medical certificate.

During the flight some passengers may feel bad. CAs must be well trained to administer first aid. That is why on board each aircraft a first – aid kit is carried. It contains a tight, a splint, syringe, cotton wool, bandages, gauze, a medicine dropper, a hot water bag and a thermometer. There are also different medicines in drops, pills, tablets or powder. Iodine, ammonia, ointment and spirit of camphor are also included into the first – aid kit.

The medicines help the CA to administer the first aid if a passenger has a terrible headache or a stomachache, a heart attack, a fit of coughing or an abscess, intoxication or bleeding. If the CA is helpless, let’s say, when a passenger has stenocardia or an awful liver attack, an attack of appendicitis or even there may be a delivery, she must inform the Captain who then requests emergency landing because of a sick passenger on board. A doctor on duty usually meets the aircraft after its landing.

Flight attendants are routinely asked the silliest questions and hear the most ridiculous complaints. • “My children are getting bored. Is there anyone in the cabin crew that can do magic tricks?” • A gentleman complained of a headache on a Milan flight and asked what the loud sound was. When told it was the jet engine he replied: “Tell the captain to turn them off because my I have a terrible headache.” He went on to threaten making an official complaint “as our terms and conditions did not state how loud the engines were during the flight”.  • One customer coming into London was worried about Heathrow being so close to Windsor Castle. He asked if cabin crew could arrange for the planes not to fly over the castle in the future in case the Queen found it a nuisance. • One stewardess heard: “Why am I getting such a poor mobile phone signal? This flight is going to be really long and I’ll be bored if I can’t use my smart phone.” • Flying into Nice, south of France, one passenger asked a stewardess if the pilot could “go a bit further down the coast so we can have a look at Monaco”.  • A group of ladies on a budget airline flight to Amsterdam kicked up a fuss because there were no free drinks. They left a comment saying: “Please could you have a wider choice of free wines, particularly champagne as this would make the journey much nicer.” • An elderly gentleman flying from Australia was disappointed that the flight attendants weren’t attractive enough. “They were much prettier in the adverts, so the flight has not lived up to my expectations,” he complained.

Ex.1 As a CA you must respond positively to complaints. First, look at three complaints during the meal service. What would you say to reply to these passengers?

Ex.3 Read and translate.

Incidents of unruly and disruptive behaviour by passengers on board an aircraft appear to be increasing. It is sometimes referred to as “air rage”. It can be also defined as any behaviour on board an aircraft which interferes with the crew in the conduct of their duties and disrupts the safe operation of an aircraft. Here are some examples.

Fighting passengers cause plane to divert. A brawl between football fans forced a plane to make an emergency landing in Germany last Friday. The men, aged 24 and 36, had been drinking and could not be restrained by cabin crew after they started fighting.

Vegas flight lands in Denver after air rage incident. A US airliner en route to Las Vegas was diverted to Denver after a passenger tried to open an exterior door on the plane, officials say. The man had reportedly consumed alcohol before and during the flight. “There was a disturbance on board involving an unruly passenger, and the pilots decided to divert to Denver”, local police said in a statement.

Abusive flyer offloaded from aircraft. An aircraft returned to its stand after taxiing for take-off to offload a passenger who had become abusive with a cabin crew member. The CA asked him to put his seat upright, but received verbal abuse in response, said an airline official. It is not known whether the passenger was drunk. When informed of the incident, the captain decided to turn the aircraft back to the stand to offload the unruly passenger.

Woman who hit CAs jailed after forced landing of Tokyo flight. A 42 – year – old woman has been jailed after causing a trans – Pacific flight to be diverted to Anchorage, Alaska. The woman was aboard the Delta Airlines flight from Tokyo to New York when, about 45 minutes into the flight, she began assaulting a CA who was offering a beverage service. CAs tried to calm her down, but she continued screaming profanities in Japanese and hit four CAs before being handcuffed and seat-belted in her seat. She continued to struggle and unbuckled her seatbelt several times, causing CAs to set up a 30 – minute rotation to watch her. Pilots were forced to dump 70,000 pounds of jet fuel to land in Anchorage and hand her over to local police.

One too many. A passenger was arrested on Friday after his flight landed on charges that he assaulted the flight crew. The man drunk five glasses of wine after boarding the flight in Atlanta, Georgia, authorities said. He became abusive when the CAs refused to serve him the sixth drink. When the purser arrived to try to calm him down, he grabbed her arms and hands. CAs armed themselves with a fire extinguisher and positioned drinks trolley in the aisles in case the man tried to get into the cockpit.

Drunk passenger jailed after going on rampage at 30,000ft. A drunken passenger went on a rampage at 30,000 feet after cabin crew caught him trying to get into the first class section. During the nine-and-a-half hour flight, the man, who was travelling in economy class, walked up and down the aisles shouting abuse, reducing passengers to tears and causing his own daughter, aged 15, to go and sit with the crew to hide from him. He was jailed for eight months after pleading guilty to being drunk on an aircraft.

Ex.4 Study these sentences.

CP: Is he drunk?

CA: I don’t think so, but he’s very agitated and abusive, He said we were in danger and he had to fly the plane. It must be a mental health problem.

CP: Is anyone hurt?

CA: No, we’re OK. What do you want us to do with him.

CP: Secure him, away from the other passengers if you can. Get someone to stay with him until we land.

C: Right, contact the ground service and tell them that we’ve got an unruly passenger. Request a diversion to the nearest suitable airfield. Have medical and security there to meet us.

CP: Centre. Flight 547. A passenger has attempted to enter the flight deck. He also attacked the cabin crew. There are injuries. We have restrained him but we need to get him off the plane as soon as possible.

T: Flight 547. Understand you have an unlawful interference. Please say fuel and persons on board.

CP: Er…178 persons and four hours of fuel remaining. Can we descend to the nearest available aerodrome? We’ll need medical and security services ready. Flight 547.

T: Flight 547. You are approaching Korean airspace. Contact Inchon Ground Service. I’ll advise them of your situation and pass on your request.

Hello, this is Tokyo Ground Service here. We have a problem with the flight 547, we expect it in your airspace.

I: What’s the problem?

T: We had a report from the flight crew. They said a passenger had attempted to enter the flight deck. The first officer said that the crew had restrained him, but believed he was still a threat.

I: Are there any injured persons?

T: The crew told me there were injuries, but they didn’t give details.

I: Did they state intentions?

T: They asked if they could descend to the nearest aerodrome, and they said they’d need medical and security services ready.

I: Thank you. Leave it with us.
