No entry is permitted into an enclosed space until a responsible officer has tested the atmosphere, has ascertained that it is free of toxic vapours, is not deficient in Oxygen and a Work Permit (SAF 11) has

been issued, and the Enclosed Space Entry Tag posted showing “Safe

Condition”.

The Enclosed Space Entry (Form SAF 11) is to be completed and initialled as appropriate by the Officer in Charge (i.e. Chief Officer or Chief Engineer) entering the space and submitted to the Master for approval. After use the permit is to be filed by the Master.

All items on the checklist part of the entry permit should be answered with a

YES.. In any situation where an answer in the checklist is answered in the negative, then a risk assessment is to be conducted on that particular question. Only if the risk is acceptable and additional precautions are put in place as necessary can the entry continue. Suitable remarks MUST be included if a question is answered with a NO.

The Enclosed Space Entry Permit relates only to entry procedures. If any form of work or use of tools is required in the space then the companies Cold Work Permit or Hot Work Permit must also be completed and authorised by the Master and Company as appropriate.

• A copy of the Work Permit is to be posted up at the access to the space

being entered.

• The completed permits are to be retained on board for inspection

A Log Entry is to be made for each completed Permit.

Where multiple spaces are documented on a single permit, each space must be clearly marked for “Safe Condition” using the Enclosed Space Entry Tag

Once the permits have been completed, the Enclosed Entry Tags may be wiped clean and re-used.

The period of validity of an Enclosed Space Entry Permit is initially four hours. However, this may be extended by a maximum of two further periods of 4 hours each under the condition that the atmosphere is rechecked prior to each extension and recorded.

Persons Nominated to Enter Enclosed Spaces

Except in an emergency situation, not more than one officer from each department will enter an enclosed space as part of the entry team. This must be taken into account when drawing up an action plan.

Each member of the team entering the enclosed space must utilize the “Man in Tank” tags by clipping them on to the Entry Tag. Details of each team member must be provided to the OOW so that appropriate records can be made on the SAF 11 for Time In / Time Out of the space
