General and Confidential Instructions to Transplant Team Members
1. It is essential that all medical practitioners and members of the
healthcare staff uphold stringent principles of confidentiality
concerning the deceased donor and their family. Any and all actions
and communications pertaining to the transplantation procedure must
be regarded as condential, an indispensable facet throughout the
entirety of the transplant process.
2. During the actual surgical procedure, healthcare professionals including
attending surgeons, physicians, nurses, technicians, paramedics, and
administrative personnel are unequivocally prohibited from engaging
in any form of conversation within public areas such as elevators,
staircases and the facility’s cafeteria. This prohibition is in place to
safeguard the integrity and privacy of the transplantation process.
3. Every individual involved in this process is mandated to exhibit
unwavering dignity and the utmost respect for the donor’s family.
Furthermore, they are obliged to promptly address any concerns or issues
that may arise during the various stages of the transplantation procedure.
4. In the event that a patient is identied as brain-dead while within
the intensive care unit (ICU), whether by the attending physicians or
nursing staff, it is incumbent upon them to expeditiously communicate
this critical information to the designated transplant coordinator.
5. It is categorically stipulated that any involvement in the transplantation
process by unauthorized individuals or third parties is absolutely
prohibited. This stringent measure is enacted to ensure the sanctity of
the transplantation process and prevent any unwarranted interference
or breaches of condentiality.
6. Explicit directives are given to the primary attending physician and
the head of the intensive care unit, mandating that they limit their
communication with the patient’s family exclusively to matters
concerning brain death and the patient’s clinical condition. Any
discussions related to organ transplantation or the procedural aspects
thereof are to be refrained from.
7. The pivotal duty of assessing the patient’s clinical status, appraising
the situational context, and determining the viability of organ retrieval

rests squarely on the shoulders of the Transplant coordinator. This
essential role involves a comprehensive evaluation to ascertain the
appropriateness and potential success of organ procurement in the
given context.
8. Subsequent to a meticulous and comprehensive assessment of the
feasibility of the transplant procedure, the designated transplant
coordinator is obligated to promptly apprise the appropriate governing
body.
9. Sole authorization to initiate the organ donation process and to engage
in communication with the pertinent organ procurement authorities
and national network of organ sharing ofcials is vested solely in the
transplant coordinator.
10. It is incumbent upon the transplant coordinator to disseminate
information to the core transplant team only subsequent to obtaining
explicit consent from the family of the deceased individual.
11. Following the initial apnoea test, it is imperative for the transplant
coordinator to expeditiously notify various departments, including
the Blood Bank, Central Laboratory, Operating Theatre, Pharmacy,
Ambulance Services, Billing Department and Transport Department.
12. Consultants are earnestly requested to accord precedence to this
process and promptly adhere to the directives issued by the transplant
coordinator.
13. In the event of the unavailability of consultants, the respective
department heads or senior consultants are kindly urged to delegate
their deputy or the next-in-command to execute the process without
any unwarranted delay or impediment. However, it is imperative that
each department’s head formulates their own strategy to ensure the
accomplishment of the prescribed objectives. The transplant coordinator
should not bear the sole burden of responsibility at any juncture.
14. The Head of the Radiology Department is tasked with the responsibility
of designating senior sonologists and skilled radiographers to offer
bedside support as necessitated during the transplantation procedure.
15. The Head of the Department of Pathology and Immunology is tasked
with the responsibility of deputing experienced senior laboratory

technicians to expeditiously orchestrate the process of tissue cross-
matching following the reception of notications from the transplant
team. Furthermore, it is imperative to ensure that pathologists are
promptly apprised of any instances involving exceptionally urgent
and critical biopsy procedures.
16. It is imperative that the clinical laboratory work round-the-clock,
continuous coverage to cater to the needs of the transplant team.
Personnel involved in this endeavour are expected to exhibit prociency
not only in performing routine laboratory tests but should also remain
prepared to facilitate the external procurement of specialized tests
when the need arises. This approach ensures the seamless provision
of essential diagnostic services for the transplant program, bolstering
its efciency and effectiveness in patient care.
17. Following the initial apnoea test, it is imperative that the transplant
coordinator promptly noties the Department of Anaesthesia. The
Anaesthetic team, inclusive of the scrub nurse, floor nurse, and
operating theatre (OT) technicians, should be on standby with essential
equipment and an adequate supply of sterile linen.
18. The nursing supervisor responsible for overseeing the operations
within the operating theatre is assigned the crucial responsibility of
maintaining an adequate stockpile of disposable surgical gowns. This
inventory must be sufciently extensive to accommodate surgeons
from other healthcare institutions who are essential members of the
organ harvesting team. This measure ensures that all participating
surgeons have access to the requisite sterile attire, fostering a seamless
collaborative environment during the organ procurement process.
19. The Blood Bank Medical Ofcer and the accompanying technicians
must be prepared to conduct necessary tests and administer blood
transfusions as required. The In-charge Medical Ofcer must establish a
predened protocol and diligently adhere to it upon receiving pertinent
information from the transplant coordinator.
20. Personnel within the pharmacy department should maintain a
baseline inventory of essential medications, immunosuppressants,
and intravenous infusions at all times and expedite procurement of
additional supplies as deemed necessary to facilitate the transplantation
process seamlessly.

21. The institution shall bear the scal responsibility for all costs incurred
throughout the entire continuum of the procedure, commencing with
the initiation of the initial apnoea test and persisting until the moment
of informing the pertinent family members. It is important to note that
this nancial arrangement may exhibit variability contingent upon the
specic policies and practices of each respective healthcare institution.
22. Both the medical and administrative teams retain the prerogative to
request prior authorization from the Head of the Institution to full
their respective roles and responsibilities during the organ retrieval
procedure.
23. The Department of Transportation bears the critical responsibility of
promptly coordinating the preparedness of essential transportation
assets, including ambulances, passenger vehicles, and potentially
even air ambulance resources such as helicopters, immediately upon
receiving the notication. Furthermore, it is essential to acknowledge
the potential requirement for the transportation of solid organs
obtained from external healthcare facilities, which mandates meticulous
logistical planning and precise coordination efforts.
24. The Head of Transportation is expected to possess an intricate
understanding of implementing expeditious methods, such as air
ambulance utilization or the establishment of rapid-access green
corridors, in collaboration with relevant state and local traffic
enforcement authorities. These measures are essential to ensure the
swift and unimpeded transport of vital medical resources during the
organ retrieval and transplantation process.
25. The transplant coordinator is mandated to conduct regular and
systematic assessments of the organ preservation materials, including
various solutions and the freezer box, to ensure their optimal
functionality and readiness for use in the organ transplantation
process.
26. The transplant coordinator bears the responsibility of formulating
a comprehensive protocol and checklist, incorporating all the
aforementioned directives and guidelines. This protocol should
be subject to periodic review and updates as dictated by evolving
circumstances and requirements.

27. The transplant coordinator assumes the pivotal role of serving as the
exclusive liaison ofcer for both the cadaver donor program and the
orchestration of organ transplantations. This entails coordinating and
facilitating all necessary communication and logistics pertaining to
these critical medical processes.
28. The senior billing clerks and personnel responsible for financial
matters are expected to ensure their accessibility and readiness,
notwithstanding their regular duty schedules and any time constraints.
The accounts department is anticipated to exercise sound judgment and
take on the responsibility of facilitating nancial support arrangements
and processing billing procedures, even during unconventional hours
such as late at night. The provision of nancial services is mandated
to operate seamlessly around the clock, 24/7, to address the exigent
demands of the healthcare institution’s operations.
29. The individuals responsible for Insurance matters must be prepared
to handle emergent situations and possess a comprehensive
understanding of the critical complexities inherent in the medical
procedures. This insurance coordinator serves as the representative
for managing interactions with insurance providers on behalf of the
patients, with the primary aim of precluding any customary and
unjustied inquiries that may be raised by the insurance companies.
Moreover, the insurance coordinator has the potential to alleviate
the nancial burden borne by the families of the organ transplant
recipients. The entire accounting team should be nely attuned to
promptly address any unforeseen nancial exigencies that may arise
during the nal moments of the transplant process.
30. The hospital administrators are obligated to offer their unwavering
support, extending their efforts even beyond their regular working
hours and capabilities. Their involvement is strictly conned to crisis
management in the event of any unforeseen complications arising
during the organ transplant procedure. The presence of administrators
and their unconditioned support serves to fortify the transplant team’s
resolve and effectiveness.
These guidelines bear profound importance, not just in safeguarding
condentiality and facilitating seamless coordination, but also in achieving

the pinnacle of success in the noble endeavour of organ donation and
transplantation. They remind us of the sacredness that underlies every
step of this journey. It is a journey where every action and process, at each
stage, calls for the divine blessings of the Almighty—a poignant reection
of the enduring faith and hope that sustains us in the pursuit of saving
lives through the seless act of organ transplantation.
Dr Gopi Ayyaswamy
The government’s initiative to bestow state honours upon organ donors is
a commendable and forward-thinking approach to addressing the critical
issue of organ shortages and promoting the noble act of organ donation.
This initiative carries numerous benets for society as a whole.
First and foremost, by recognizing organ donors with state honours, the
government is sending a powerful message about the importance of organ
donation. It underscores the selessness and life-saving potential of this
act, thereby increasing public awareness. This increased awareness can
lead to more informed discussions within families and communities about
organ donation, potentially resulting in a greater number of individuals
choosing to become donors.
The recognition of organ donors can help break down barriers and
reduce stigmas associated with organ donation. Cultural and religious
beliefs often play a signicant role in people’s decisions regarding organ
donation. When the government acknowledges and celebrates donors,
it can help bridge the gap between these beliefs and the pressing need
for organ transplantation. This can foster a more inclusive and accepting
attitude towards organ donation, ultimately beneting patients in need.
Addressing the shortage of available organs for transplantation is a
critical and ongoing challenge. By actively promoting organ donation
and honouring donors, the government is taking a proactive step
toward increasing the supply of organs. This, in turn, offers hope and
a lifeline to those individuals who are anxiously waiting for life-saving
transplants.
