Foreword

In my many years in cardiovascular medicine, I have seen how critically
important education and team care are to realizing the promise of our rapidly
advancing technologies and practices, and how hard it can be to deliver
needed content in meaningful and accessible ways to the right audiences.
Multidisciplinary topics or aspects of medicine requiring team care are a particularly challenging area as expertise is often siloed, yet team members need
to be familiar with their colleagues’ expertise to provide the most effective
and collaborative care. Structural heart interventions are a perfect example of
this: structural heart experts, proceduralists, imagers, intensivists are collectively essential to optimal patient care. I recognized this at the beginning of
the structural heart intervention boom in the mid-2000s, as the imaging lead
investigator in the rst randomized trials leading to the initial approvals of
two of the most important structural heart device advances: transcatheter
heart valves, in the aptly named PARTNER trials using balloon inatable
valves, and the PROTECT and PREVAIL left atrial appendage occlusion trials. In both sets of landmark trials, the imaging teams provided important and
powerful input into patient selection, procedural conduct, and long-term success. In turn, the trials provided an indispensable learning environment for
imagers (as well as implanters) as we adapted old techniques and learned new
ones to better support the heart team and our patients. Some of these insights
are still state of the art; for others, these early innovations were foundational
as imaging capabilities and skills have advanced to keep pace with rapidly
evolving technologies and techniques.
Since then, the need for cross-specialty education in structural heart disease has only grown with the tremendous growth and diversity of procedures
and capabilities. In writing Cardiac Imaging in Structural Heart Disease
Interventions: A Textbook for the Heart Team, long-term esteemed colleagues
Drs Kelsey, Vemulapalli, and Sadeghpour have identied a critical gap in
educational materials targeting those who may not otherwise be exposed to
imaging content but whose daily work would benet immensely from more
in-depth knowledge. By mixing together case studies, highlighting key
points, providing summary tables and carefully targeted gures, and ending
with board type questions, the authors have made the content highly accessible to those with different learning styles or needs.
This book is set apart and elevated by the decades of experience as imagers and educators of each of the editors and authors. Suitable for trainees and
non-imaging experts as well as imagers, this is a book to read through to learn

about the state of the art, turning often to the marvelous images and gures in
the text and online. In this sense, it is a book about the past and the present.
However, it is also a book about the future. It is a reference text to keep handy
for years to access a table or gure to shed light on a specic issue, or to ip
through to answer a specic question. Most importantly, it is a foundation and
inspiration to ongoing advances in imaging and in structural heart disease.

Foreword

Preface

Structural heart interventions require a team of medical professionals, excellent communication, and detailed cardiac imaging for successful, safe performance of these complex procedures. To facilitate effective communication
and successful procedures, a clear understanding of the value of cardiac
imaging is required by all members of the multidisciplinary team including
cardiothoracic and vascular surgeons, interventional and noninvasive cardiologists, cardiac anesthesiologist, research and clinical nurses, sonographers,
cardiovascular/OR technicians, and cardiology/cardiac surgery/cardiac anesthesia fellows. While each brings valuable skills to the team, a shared understanding of the importance and role of imaging is critical to the optimal
teamwork and communication necessary for the success of these
interventions.
While many great textbooks have addressed different aspects of structural
heart disease interventions, few focus on the integration of cardiac imaging;
and if cardiac imaging is the focus, the text is directed towards cardiac imagers. We have written this textbook with several features leveraging the tenets
of adult learning to provide digestible information accessible to all heart team
members:
• Clearly stated learning objectives for each chapter
• Case-based format with emphasis on images and multi-media
• Special emphasis on cardiac imaging within the preprocedural, intraproce-
dural, and postprocedural heart team discussions
• Comparisons of the additive value of multi-modality cardiovascular
imaging
• Imaging pearls for non-imagers
• Key points about the value of each imaging modality and complexities at
each stage of the interventional procedure.
• Board-style questions with explanations
We are deeply indebted to the many authors who have contributed their
expertise and invaluable perspective in the chapters of this textbook. They
have given you, the structural heart team members, a wealth of cardiac imaging insights acquired over many years of experience in the growing eld of
structural heart interventions.
We hope that you will nd this textbook a valuable resource for your structural heart team, offering insights and clinical pearls that will prove

instrumental in your team’s success in providing these procedures for your
cardiac patients.
