Conclusion

This is a false positive glaucoma. The real diagnosis is congenital bilateral megalopapilla without glaucoma.

The study of primary congenital glaucoma has shown that a congenital anomaly of the pretrabecular chamber angle leads to ocular hypertension. If its surgical treatment does not normalize the intraocular pressure it leads to glaucomatous optic neuropathy, identical to that found in adults, but with a much faster development and identical defects in the visual field. In the literature on congenital glaucomas, neither the risk factors nor the normal-tension glaucomas are described.

This simple summary should indicate that in the future, when 24-h IOP monitoring in open-angle

glaucomas would be possible, the concept of glaucoma may change. Ocular pressure will not be seen as a risk factor, but rather the cause of glaucomatous optic neuropathy.

Currently, 24-h IOP monitoring is not possible, because it is very inconvenient for the doctor to take the pressure at 6 a.m. in the morning or because hospitals do not pay for DPC. In the near future, we will have a new automatic device that will enable such monitoring.
