Healing of HSV Epithelial Keratitis with Acyclovir Ointment:

A Treatment Failure

Fig. 2.12  This HSV lesion, (presumably) treated with acyclovir ointment for a week shows typical features of ongoing infection: large numbers of clearly visible “bright” swollen/rounded cells (white arrowhead) surrounded by “dull” ones (black arrowhead)

Fig. 2.13  The same lesion as in Fig. 2.12. Tear fluid stained green with fluorescein sodium is pooling in surface depressions (black arrow); elevated areas appear dark (white arrows). Some swollen cells are visible in the lower part of the lesion (arrowhead) (the upper part is slightly out of focus)

Case Report

The patient was a 34-year-old woman with recurrent HSV infections around her left eye. She did not recall any previous eye involvement. Probably, she

had not used the ointment properly. She was instructed how to apply it, and a week later all signs of active infection were gone.
