in multiple tunnels. Another important detail in the injection technique involves creating a tunnel on insertion and injecting only during withdrawal of the cannula (Fig. 23.7). There is variability with regard to cannula size and other alternative techniques believed to improve graft success. Many authors have described using a 14-gauge blunt tip or curved microcannula for injection, whereas others have used a 2–3 mm cannula for reinjection.

The common principle in the injection technique is to maximize the fat tissue surface area that is in contact

with well-vascularized tissue. The author uses the “Pearl” technique by using droplets of fat in a multilayer fashion starting from the periosteum upward to avoid irregularity (Figs. 23.823.11).

23.5 Complications

Complications include swelling, lumpiness, infection, pain, discomfort, and unsatisfactory long-term results.
