Case 6: Adenovirus or HSV Epithelial Keratitis?

Case Report

A 73-year-old man with irritation and redness in the right eye. The duration of symptoms was one week.

The right eye was injected, and the cornea showed branching figures reminiscent of HSV. The epithelium

healed within 10 days. Virus isolation test showed adenovirus; serotyping was not done.

The photographs of the right cornea were taken one week after symptom onset.

Fig. 3.11 (a–c) Adenovirus epithelial keratitis, 1 week after symptom onset. (a) The epithelium shows branching figures reminiscent of HSV dendritic keratitis. The resemblance is only superficial. The substructure shows features compatible with adenovirus infection: (b) rounded/abnormal cells (white arrowhead) and (c) many cyst-like structures (black arrowhead)

Fig. 3.12 (a–c) (Opposite page) Adenovirus epithelial keratitis. One week after symptom onset, the cornea shows many epithelial infiltrates (straight arrows) which (a) are light-reflecting, (b) have granular appearance, and (c) show a circumscript green fluorescein staining and adjacent surface elevations (dark; bowed arrow). In all photographs are visible rounded/abnormal cells (white arrowheads) and in (b) also cyst-like structures (black arrowhead). (The straight arrows are placed in corresponding locations)
