Executive Summary
This is like an abstract – it tells what the problem is, why a report is being written and what the outcomes are. A busy executive should be able to read this and know what the report is about instantly and should then use the rest of the report as backup.

This could be the approach taken, one of the problems or one of the solutions depending on the question.

You don’t have to have sub points – it depends on the type of information you have and how you want to display it.

Recommendations
List suggestions and recommendations here. Language can be strong – (e.g. must) if it is vital, but otherwise usually should/could is used as it is up to a higher power to decide whether to implement these recommendations.
Conclusion
Not all reports need a conclusion or recommendations. It depends what type of report it is. If it is an informative report, then it will probably only have a conclusion recapping the main ideas, but if it is an analytical report then it will probably list recommendations and may not have a conclusion.
