5. Committee composition: a presiding commissioner is a public servant of State Inspection on labour precaution, representative of the organ to which the enterprise belongs, representative of the State Sanitary and Epidemiological service and technical experts.

1.Всі нещасні випадки захворювання і аварії на виробництві підлягають розслідуванню відповідно до ДНАОП 0.00-4.03-01.

2. Комісія при загальному розслідуванні: керівник структурного підрозділу, він-же голова комісії представник профспілкової організації, свідки, технічні експерти.

3. Спеціальному розслідуванню підлягають: нещасні випадки зі смертельним наслідком, групові нещасні випадки, які сталися одночасно з двома і більше працівниками незалежно від тяжкості ушкодження їх здоров’я, у випадку смерті на підприємстві, випадки зникнення працівника під час виконання ним трудових обов’язків.

4. Комісія при спеціальному розслідуванні призначається наказом керівника територіальному органу Держогляд охорони праці.

5. Склад комісії: голова комісії-посадова особа органу державного нагляду за охороною праці, представник органу до якого належить підприємство, представник державної санітарно-епідеміологічної служби, технічні експерти.
