Я, Иванов Иван Иванович, ХХ. ХХ. ХХХХ года рождения, проживающий в городе Омске, по ул.ХХХХХХХХХХХХХ, в доме ХХ, кв. ХХ(паспорт ХХ ХХ ХХХХХХ, выдан УВД № 2 ЦАО г. Омска 17.05.02 г.), настоящим завещанием делаю следующее распоряжение:

1. Все мое имущество, какое ко дню моей смерти окажется мне принадлежащим, в чем бы таковое ни заключалось и где бы оно ни находилось, я завещаю ХХХХХХХХХХХХХ ХХХХХХХ ХХХХХХХХ ХХХХ г.р.

2. Содержание статьи 1149 Гражданского кодекса Российской Федерации мне нотариусом разъяснено.

3. Текст завещания записан нотариусом с моих слов и до его подписания прочитан мною лично в присутствии нотариуса.

4. Настоящее завещание составлено в двух экземплярах, каждый из которых собственноручно подписан завещателем. Один экземпляр завещания хранится в делах нотариуса города Омска Ивановой М.С., а другой экземпляр выдается завещателю Иванову Ивану Ивановичу.

2. Содержание статьи 1149 Гражданского кодекса Российской Федерации мне известно.

4. Настоящее завещание составлено в двух экземплярах, каждый из которых собственноручно подписан завещателем. Один экземпляр завещания хранится в делах нотариуса города Екатеринбурга Ивановой М.С., а другой экземпляр выдается завещателю Петрову Петру Петровичу.

Employment Agreement

The below signed, _______________ , employer and ___________________, employee, having negotiated in good faith and with good and sufficient consideration being exchanged, hereby reach and enter into this agreement as to the employee’s terms of employment by employer, with the same being determent to be as follows:

The above listed responsibilities and duties may be modified by the Employer upon reasonable notification to the Employee, however drastic change of duties and responsibilities by the Employer is cause to Employee to terminate this Agreement upon giving two weeks notice of his intent to do the same.

Employee owes the following additional duties and obligations to the Employer:

• Employee shall expend his/her best, full time efforts on the Employer’s business, and shall not engage in any competing business activity that impedes Employee from performing as stated;

• Employee shall not disclose any of the Employer’s trade secrets or proprietary information to any other party.

• Employee shall refrain from working for, advising or consulting and /or starting up or participating in any business of any kind that performs work or conducts business in the same trade, or field as that in Employer is involved for a period of _____________ years after termination or nonrenewal of this contract.

Employer owes the following additional duties and obligations to Employee:

• Employer shall reimburse all of the Employee’s business related expenses within ____ days after receipt of company invoice for the same.

• This agreement shall be assumed by and be binding upon the Employer’s successors and assignees.

The following additional terms and conditions are agreed to by the parties:

• Any dispute arising from, or concerning this Agreement of Employment shall be resolved by submission of the same to the binding arbitration with GAMA, Inc., or Georgia. In so agreeing, the parties expressly waive their right, if any, to the jury trial on said issues and further agree to follow GAMA’s Rules of Arbitration and that the award of the Arbitrator shall be binding upon them as if rendered by a court of law. It is further agreed that said award shall be enforceable in any court having jurisdiction over the same, with the costs of enforcement being born by the noncomplying party.

• This agreement is severable, so that if any provision herein should be held by a court of law to be invalid, all other provisions shall be continue in force.

• Waiver by the parties of any provision herein shall not constitute waiver of the right to enforce this agreement.

• This agreement constitutes the entire agreement of the parties, and supersedes any prior agreement. Moreover, no oral modifications shall be permitted to vary or change the terms herein stated.

• Termination of this contract may be made by Employer without cause and for any reason. On the date of termination, Employer shall provide a severance allowance of ___________ of gross minus the statutorily required withholdings.
