Classifying mental disorders

The ways in which we decide what is and what is not a mental disorder are by no means settled. But no less controversial is the issue of classifying these disorders. How do we decide whether two patients have the same disorder or two different ones?

To answer this question, practitioners have tried to set up classificatory schemes for mental disorders analogous to the diagnostic system in other branches of medicine. Here, as elsewhere in science, the purpose is to bring some order.

The diagnostic process begins with a consideration of the symptoms. Examples of symptoms in psychopathology would be anxiety or an intense sense of being worthless. The trouble is that an individual symptom is rarely enough for diagnosis; anxiety is found in many different mental disorders. As a result, the diagnostician often looks for a pattern of symptoms that tend to go together, a so-called syndrome. An example of such a syndrome in psychopathology is a set of symptoms that includes disorganization of thinking, withdrawal from others, and hallucinations. This syndrome is characteristic of schizophrenia.

A great amount of conditions are included within psychiatric classification schemes. Some are somatogenic disorders, while others seem to be psychogenic. Still others are between criminality and psychopathology, or between normality and psychopathology. Still others may simply be deviant behavior patterns that our own society doesn’t approve of.

By grouping of this kind, psychiatrists have set up an order of mental disorders that includes mental deficiency, senile deterioration, schizophrenia, disorders such as mania and depression, various conditions that are covered by a general name of neurosis (for example, phobias, obsessive-compulsive disorders), psychophysiological disorders such as hypertension produced by emotional stress, personality disorders such as antisocial personality, alcoholism and drug addiction, and various forms of sexual deviation.
